It doesn't seem that this file is changed when the users hides the package:
marcot@zezinho:~/.cabal/tmp$ cp /var/lib/ghc-6.12.1/package.conf.d/mtl-1.1.0.2.conf ~/.cabal/tmp
marcot@zezinho:~/.cabal/tmp$ sudo ghc-pkg hide mtl
marcot@zezinho:~/.cabal/tmp$ diff /var/lib/ghc-6.12.1/package.conf.d/mtl-1.1.0.2.conf mtl-1.1.0.2.conf
marcot@zezinho:~/.cabal/tmp$
When did you notice a change in this file?
