exim4-config from testing/unstable only listens on localhost by default.

This points to the problem that the first two options ("internet site;
mail is sent and received directly using SMTP" and "mail sent by
smarthost; received via SMTP or fetchmail") are deceptive, and probably
contribute to people not having working outgoing mail.  They should make
it clear that SMTP won't be open to the world by default.

With this clarification, I think that "internet site" is a sensible
default.  Despite firewalls and blacklists, this does work most of the
time.
