It's d-i trunk revision 51621 with this sources.list.local:

deb copy:/home/guido/debian-installer/d-i/installer/build/ localudebs/
deb http://ftp.de.debian.org/debian testing main/debian-installer

The localudebs are:

partman-utils
partman-base
libparted1.7-udeb
parted-udeb
partman-multipath
disk-detect
partman-auto
grub-installer

with the multipath patches posted to the BTS.
