Bug#380648: installation report

Package: installation-reports

Boot method: netinst CD
Image version: etch daily image 31. Aug 2006
Date: 20060731 18:00

Machine: Custom dual Athlon MP with Tyan mainboard
Processor: Athlon MP 2000+
Memory: 1.5G
Partitions:
2x18G SCSI disks, each with 17G RAID partition and 1G swap.
both RAID partitions merged together as md0 mirror. LVM was set up on md0, containing only one logical volume that was mounted as / with ext3 filesystem.

Output of lspci and lspci -n:
Not currently available because no bootable system was installed. AMD 760MP chipset, AMD8111 IDE with cdrom, Adaptec scsi, 2x3Com fast ethernet, ATI graphics.

Base System Installation Checklist:
[O] = OK, [E] = Error (please elaborate below), [ ] = didn't try it

Initial boot worked: [O]
Configure network HW: [O]
Config network: [O]
Detect CD: [O]
Load installer modules: [O]
Detect hard drives: [O]
Partition hard drives: [O]
Create file systems: [O]
Mount partitions: [O]
Install base system: [O]
Install boot loader: [O]
Reboot: [E]

Comments/Problems:

Install worked fine but arter reboot the machine did not come up.
First it found the hardware OK, activated md0 with 2 out of 2 drives.

Then a line like this (maybe harmless):
Failure: old RAID initialisation script found, getting out of its way...

Then device-mapper was initialized and tol that it has 1 logical volume
active.

Then:
Done.
stdin: error 0
Begin: Running /scripts/local-premount ...
Done.

Then an error from modprobe:

Usage: modprobe [-v] ...
mount: Cannot read /etc/fstab: No such file or directory
Begin: Running /scripts/local-bottom ...
Done.
Done.
Begin: Running /scripts/init-bottom ...
mount: Mounting /root/dev on /dev/.static/dev failed: No such file or directory
Done.
mount: Mounting /sys on /root/sys failed: No such file or directory
mount: Mounting /proc on /root/proc failed: No such file or directory
Target filesystem doesn't have /sbin/init

and then I get busybox shell from initramfs.
