following the latest instructions from the bug report:

$ cp dir.ok dir
$ ginstall-info /usr/share/info/maplev.gz dir
now the dir file broken.
"
with the files attached there, I think the bug is still there.
Notes:
- i did only test with debian/stretch
install-info 6.3.0.dfsg.1-1+b2
