Could you provide

 (1) output from

	/usr/share/bug/linux-image-$(uname -r)/script 3>&1

     (which includes basic information about your hardware)

 (2) output from running "dmesg" after booting with a kernel that
     triggers this problem (you can get it over ssh, for example)
