I expect this would be tricky to do reliably using iptables, since
iptables cannot easily determine the contents of an email message:
remember that iptables deals with packets.

Why not make use of the facilities in qmail to do this blocking?  And
only allow outgoing SMTP from the mail server (which you *can* enforce
using iptables, of course).
